Your goal. Your opportunity. Your growth.
Set a practical enrollment goal, identify the Medicare patient population you want your partnerships to reach, and explore what achieving your goal could mean financially.
Your numbers will look different from someone else's, and that's expected. This exercise gives you a starting target. The Provider Plug lessons explain how to build engaged partnerships, create thoughtful campaigns, and revisit your strategy every 90 days.
How many enrollments are you working toward?
Choose a timeframe and enter your enrollment goal. Your 12-month goal and Medicare patient-panel target will update automatically.
The 30× approach: Based on my experience, I start with the number of enrollments I want over 12 months and multiply it by 30. That gives me a target for the combined Medicare patient panels across the provider relationships I'm developing. It's a planning method, not a guaranteed conversion rate.
What do you earn per enrollment?
Enter what you receive, whether that's direct commission, an LOA share, or an agency/FMO/SGA override. There is no preset CMS rate.
Example: if your agency receives a $75 override per enrollment, enter 75. If your own commission is $180, enter 180.
For the 12-month goal, enter how many enrollments you expect to complete yourself and how many your other agents will complete. This avoids treating the same enrollment as two separate sales.
If an override is also paid on your personal sales, include that amount in your personal earnings field. Use only distinct enrollments within the goal.
Your earnings are an illustrative gross compensation scenario, not your net profit or financial ROI. Actual payment depends on your contract, eligibility, enrollment status and applicable adjustments.
Combined Medicare patients to target
Enter an enrollment goal to see your suggested combined Medicare patient-panel target.
Medicare patient panel = Medicare patients associated with a provider practice, not the practice's entire patient population. Add together reasonably distinct Medicare patient panels across your offices. Patients in multiple offices should not be counted twice.
Your potential annual gross earnings
Enter your goal and earnings information to see what reaching your goal could produce.
Illustration only. Not guaranteed earnings or true ROI. Excludes expenses, taxes, renewals, ancillary revenue, chargebacks, unpaid enrollments and other adjustments.

My quarterly partnership check-in
Building integrated provider relationships takes time. Record what actually happened, then decide whether you need to increase campaigns, re-engage an office, improve follow-up, or add relationships. Your goal is a guide, not an expectation that every quarter will perform equally.

Personal planning tool. Combined Medicare panels, not entire patient panels. The 30× multiplier is an experience-based planning approach, not an expected conversion rate. Compensation projections assume the enrollment goal is met and applicable payments are earned; not guaranteed, net profit or ROI.
